Results from a survey conducted by Kantar Health on behalf of SK Life Science, Inc., reveal that for people with epilepsy, transportation can be a significant challenge. Some people may even be restricted from driving, depending on the severity of their seizures and where they live.

We are excited to announce our newest version of the partnership with SK Life Science, Inc., for the Uber ride-share campaign. Through this updated initiative, people with epilepsy or seizures can receive one (1) $500.00 Uber ride-share voucher every twelve (12) months to help with transportation for medical services or appointments, employment travel, and other essential appointments.

Who is Eligible?

  • Be at least 18 years of age.
  • Current resident of and residing within the United States or Puerto Rico.
  • Have a diagnosis of epilepsy or a seizure condition.
  • Have an income equal to or below 400% of the federal poverty level issued by the United States Department of Health and Human Services for the current year. (Unsure whether you are at 400%? Visit the FPL Calculator.)
    • Submit your most recent federal tax return.*
  • Ability to obtain a verifiable annual letter from treating physician:
    • The letter must be addressed to the Epilepsy Foundation of America.
    • The letter must be on your physician’s official letterhead and signed by your physician.
    • The letter must indicate that you are unable to operate a motor vehicle. 
    • This letter must be dated within 30 days of the date of your Rideshare application.
  • Have a valid email address.
  • Have a valid phone number.
  • An Active Uber account, or the ability to obtain an Uber account, with a valid alternative payment method available should your ride cost exceed the cost of the voucher value.

*If you are currently unemployed, or do not file taxes, you may submit one of the following documents instead of a recent tax return. Proof of income is subject to verification:

  1. Affidavit of No Income (notarized): A sworn statement confirming that you have not received earnings during a specified period.
  2. Letters from Public Benefit Agencies: Government assistance programs generate official records verifying your financial status.
  3. Verification Through Tax Transcripts: Tax transcripts from the IRS provide an official record of reported income.
  4. Employment Verification Letter: If you have a job offer, you can provide a signed offer letter.
  5. Tax Returns: These documents can serve as proof of income.
  6. Official Statement/Letter from a CPA or Trust Manager: A professional can verify your financial status.
  7. Bank Statements: Check your bank account statements for a record of funds.
  8. College Financial Aid Documents: If you are a student, you can provide documents related to financial aid.
  9. Guarantor: If you have a guarantor, they can vouch for your financial situation by submitting a notarized affidavit about your financial status.

Terms and Conditions

  1. Individuals are only eligible for one (1) voucher every twelve (12) months while funding is available.
  2. Applications MUST be completed by individual who will be redeeming the voucher. Applications on another person's behalf will not be accepted unless your doctor's letter notes they are cognitively unable complete the application on their own.
  3. Vouchers are non-transferable. 
  4. Vouchers expire six (6) months after they are issued. For example, if you received your voucher in July 2025, then your voucher will expire at midnight on the last day of D. 
  5. Any balance on your voucher may be lost if you do not use the entire voucher balance before the expiration date. Any balance lost cannot be transferred to any future voucher or credited to your account by Uber or the Epilepsy Foundation.
  6. You are responsible for all account fees and any ride fees over the value of the voucher.

How to Apply

  1. Agreeing to the Terms and Conditions is an essential step in the application process, as it confirms your understanding of the requirements and your consent to proceed under the stated guidelines.
  2. Complete a Ride Share program application to apply for a voucher. A completed application ensures a smoother and more efficient review process. Please take care to provide all required information to help us evaluate your application promptly. 
  3. Upload a copy of your signed physician’s letter and federal tax return with your application. If you are unable to attach your documents, you can fax them to 301.731.3868. Be sure that your name matches the application and is legible. Applications received without the current year's federal tax return or a signed physician’s letter will be held for ten (10) business days. Your application will only be considered once all documentation is received. If no documentation is received, your application will be denied, and you must reapply. We are not able to accept applications or letters addressed to other programs as documentation.

The Epilepsy & Seizures Helpline monitors submissions, and an Information Specialist will respond within three business days with the status of your application.
